Job Description
A Dementia Care Coordinator plans, delivers, and monitors care and programming for older adults living with dementia. This role collaborates closely with nursing staff, caregivers, and families to ensure individualized, person-centered support that respects the unique experiences of those living with dementia. Responsibilities include coordinating daily routines and enrichment activities, promoting safety and engagement, and educating staff in best practices for dementia care.
The Dementia Care Coordinator also plays a key role in behavioral response planning, building meaningful connections with residents, and supporting family members through communication and care conferences. This position is ideal for individuals with a deep sense of empathy, strong observational and organizational skills, and a commitment to promoting dignity and quality of life.
Education Requirements
A high school diploma or GED is typically required. Many employers prefer additional education or training in nursing, social work, therapeutic recreation, gerontology, or a related field. Certification as a Certified Dementia Practitioner (CDP) or equivalent dementia-focused training is often valued. Some employers may prefer a Licensed Practical Nurse (LPN) or Registered Nurse License through the Minnesota Board of Nursing.
